Just straight up exchange of 200 Thorium Shells for 200 Thorium Headed Arrows.

This can be repeated, I'm guessing this was added in to allow bow users to get ammo comparable to Thorium Shells. They have exactly the same DPS and requirements of Thorium Shells, just an arrow instead. You can convert as many Thorium Shells into Thorium Headed Arrows as you want.

You just get the arrows and a disatisfied look on your face... No reputation gain at all. Worst use of Thorium ever.
